Nurse Practitioner

SoCal Wound Care is seeking a dedicated Nurse Practitioner to join our multidisciplinary team in San Diego, CA. This position offers a dynamic hybrid schedule combining vital clinical evaluations with work-from-home flexibility. In this role, the Nurse Practitioner will specialize in the assessment, diagnostic evaluation, and treatment of patients with a variety of acute and chronic wounds. You will develop individualized care plans, perform clinical wound debridement, and implement strategic mobility and offloading solutions. Beyond traditional treatment, you will leverage our unique clinical philosophy by incorporating nutritional interventions tailored to enhance tissue repair and optimize patient outcomes. Strong interpersonal communication skills are essential, as you will actively educate patients and empower caregivers with practical knowledge for ongoing support. Candidates must efficiently manage electronic health records (EHR) documentation and continuously adapt care strategies to meet changing patient requirements, delivering compassionate, high-quality, patient-centered care.
